The certification process facilitated by Dr. Kemal Kucuk is distinct from the approval process overseen by the Illinois Department of Public Health (IDPH). Should your application not receive approval from the IDPH, it will not impact the charges associated with the certification process.
Consultation Not a Guarantee
While scheduling a consultation with Dr. Kemal Kucuk is a crucial step in obtaining a medical cannabis card, it does not inherently ensure approval. Approval depends on meeting the specific criteria outlined by the IDPH.
Illinois Residency Requirement
To qualify for a medical cannabis card in Illinois, you must provide evidence of residency in the state. This can be demonstrated through a valid Illinois driver’s license or official mail addressed to your Illinois residence.
No Insurance Coverage
Please be aware that Dr. Kemal Kucuk does not accept insurance for the certification process. All costs related to certification are the responsibility of the patient.
Age Eligibility
There is no minimum age requirement to apply for a medical cannabis card. However, minors (individuals under 18) seeking certification will require parental consent. Additionally, a House Call Visit within the Chicago Metropolitan Area by Dr. Kemal for minors entails an additional $200 fee, making the total cost for a 3-year Certification $500.

Effective June 22, 2021, the Medical Cannabis Patient Program no longer mails medical cannabis registration cards to current patients and new applicants. Instead, these cards are accessible for printing or download within your medical cannabis patient account.
